All Financial Assistance Request Forms that are filed with American Housing Programs must be named and saved (save file as) as with the following information.

1. Loan Originators Full Name
2. Homebuyers Last name only!
3. Date: day and month

An example: Davie Jones-Russell-3-2, then save as, then email.
